Both conditions arise from impaired digestion (Mandagni) producing Ama, which then localises in joint spaces. The treatment approach differs significantly — Ama Vata requires initial detoxification and anti-inflammatory therapy, while Sandhigata Vata needs deep nourishment and oleation from the start. This precise differential diagnosis is why Ayurvedic treatment for arthritis is more effective than a one-size-fits-all approach.
For Ama Vata (inflammatory arthritis): treatment starts with Langhana (therapeutic fasting) and Deepana (digestive fire kindling) to reduce Ama. Once Ama is reduced, Virechan (purgation) clears Pitta-related inflammation. Valuka Pottali Sweda (hot sand bolus fomentation) provides dry heat that reduces swelling without aggravating Ama. For Sandhigata Vata (degenerative arthritis): Janu Basti (warm medicated oil pooled over knees using a dough dam) deeply nourishes cartilage. Pinda Sweda with Shashtika Shali (medicated rice bolus) provides simultaneous warmth and nutrition. Basti therapy (medicated enema with Dashamoola and sesame oil) addresses the root Vata imbalance systemically.
Inflammatory markers and joint swelling typically reduce within 10-14 days. Morning stiffness decreases from 30+ minutes to under 10 minutes within the first week. Joint mobility improves progressively over 3-4 weeks. With consistent treatment and lifestyle changes, many patients reduce or eliminate dependency on NSAIDs within 2-3 months.
